If you don't have a reason code, please select "No ORC - Other." This applies to BCI background checks only.
If you do not have a reason code, please contact the agency, organization, or individual who referred you for the background check. We cannot provide you with a code or process your fingerprints until you provide us with the correct ORC code.
Consent and Terms Agreement By accessing, entering, scheduling, requesting, utilizing, or otherwise engaging with any fingerprinting and background check service provided by Ohio Security & Investigations, LLC or 10-8 Protective Services, LLC, each client—along with any individuals who accompany, represent, or act on their behalf—acknowledges that they have thoroughly reviewed, fully comprehended, and voluntarily accepted all contractual terms, operational procedures, service conditions, regulatory disclosures, liability clauses, indemnification agreements, and billing policies contained within this document. This consent is legally binding and applies universally across all stages of engagement—including but not limited to intake coordination, appointment booking, documentation submission, biometric processing, facility entry, data transmission, and post-service verification. The client, by continuing with any interaction or transaction, expressly waives any claim of ignorance, misunderstanding, or nonconsent regarding these policies. 1. Scope of Fingerprint Capture and Submission Process Fingerprinting services are executed exclusively for lawful, regulatory, and compliance-based purposes. Depending on the nature of the request and agency protocol, clients may be processed using traditional ink techniques or digital live scan technology. All captured biometric data is submitted only to the designated government entities: the Ohio Bureau of Criminal Investigation (BCI) and/or the Federal Bureau of Investigation (FBI). We do not transmit, replicate, or distribute fingerprint records to any third party, private organization, or outside agency for any reason. All submissions are securely transmitted in full accordance with applicable state and federal laws governing data privacy and regulatory compliance. 2. Purpose and Regulatory Justification for Background Screening Fingerprint-based criminal background checks form an essential component of state and federal compliance practices. These screenings support employment qualification, professional licensing, volunteer certification, legal credentialing, and security clearance procedures across high-trust sectors, including education, childcare, law enforcement, healthcare, financial regulation, judicial services, public administration, and private security. Ohio Security & Investigations, LLC and 10-8 Protective Services, LLC serve as third-party facilitators and are not responsible for determining specific requirements mandated by the requesting organization. It is the sole responsibility of the client to confirm submission protocols—including preferred method (ink or live scan), agency-specific codes, required supporting documents, and time-sensitive deadlines—prior to scheduling a fingerprint appointment. 3. Identification Requirements for Service Eligibility All individuals scheduled for fingerprinting services must present a valid, unexpired, government-issued photo ID. Acceptable forms of identification include a state-issued driver’s license or ID card, a U.S. passport, federal or state military identification, or other officially recognized credentials. Failure to present proper identification at the time of service will result in denial of fingerprinting, forfeiture of the appointment slot, and imposition of financial penalties as outlined in Section 4. 4. Appointment, Rescheduling, and Financial Responsibility Policy Appointments are considered binding service commitments and are reserved for the exclusive use of the individual at the scheduled date and time. Clients are expected to arrive promptly, prepared with all required identification and documentation. Any client arriving more than ten minutes late, failing to appear without prior notice, or unable to comply with intake requirements may be rescheduled at the discretion of Ohio Security & Investigations, LLC or 10-8 Protective Services, LLC. A rescheduling fee of $25 will apply to any appointment that must be rescheduled due to client tardiness, no-show status, failure to complete online payment, or lack of appropriate documentation. Furthermore, in cases where an appointment is missed and payment was not made online in advance, the client agrees to a billing charge of $40 to cover administrative costs, lost personnel time, and operational resource allocation. This fee is enforceable and payable prior to rebooking or receiving any future services. Unpaid fees may result in suspension of future scheduling privileges or collection action. 5. Refund and Reprint Policy All fingerprinting and background check services provided through Ohio Security & Investigations, LLC or 10-8 Protective Services, LLC are strictly nonrefundable. Once fingerprint data is captured and transmitted to the designated processing agencies (BCI and/or FBI), the transaction is considered fulfilled. Refunds will not be issued for rejection by the receiving agency, delays caused by outside systems, processing errors unrelated to our staff, or dissatisfaction with submission outcomes. Clients requesting reprints due to illegible fingerprints or agency-specific resubmission demands must pay an additional reprint fee and schedule a new appointment. We strongly recommend that clients confirm procedural requirements with their respective agencies prior to initiating service. 6. Client Accuracy and Responsibility Statement The client affirms that all personal information submitted at the time of service—including legal name, birth date, residential address, Social Security number (if applicable), agency codes, and documentation—is complete, accurate, and truthful to the best of their knowledge. Any false, misleading, or incomplete data may result in rejection by the receiving agency, additional service charges, or legal liability. Ohio Security & Investigations, LLC and 10-8 Protective Services, LLC are not responsible for errors arising from incorrect client-supplied information. 7. On-Site Health and Safety Requirements To maintain a safe and sanitary environment for all clients and staff, visitors must comply with posted safety guidelines and verbal instructions from staff. These may include use of hand sanitizers, masks, distancing protocols, and restrictions on the number of accompanying guests. Any visitor showing visible symptoms of illness or who has been recently exposed to a contagious condition may be denied entry and asked to reschedule at a later date. 8. Entry Agreement and Health Liability Clause By entering the physical premises of Ohio Security & Investigations, LLC or 10-8 Protective Services, LLC, clients and accompanying persons agree to comply with all health and safety guidelines, conduct expectations, and intake protocols. Any individual who knowingly enters while ill, symptomatic, or contagious shall be held personally liable for any resulting exposure, service disruption, financial loss, or harm to company reputation. Damages may include loss of wages, client cancellations, sanitation expenses, temporary closure, and reputational harm. Clients expressly agree to indemnify and hold harmless both companies from any claims arising from illness-related incidents, negligence, or violations of health protocol. 9. Premises Liability and Risk Assumption Agreement By physically entering our location, clients and guests accept full responsibility for any risks encountered while on site. This includes but is not limited to slips, trips, accidental injuries, and general liability for physical incidents occurring on or around the property. Entry constitutes release of liability for Ohio Security & Investigations, LLC and 10-8 Protective Services, LLC from all personal injury claims unless otherwise mandated by law. 10. Service Refusal and Behavioral Policy We reserve the right to refuse service at any time to any individual who exhibits disruptive behavior, fails to provide accurate documentation, engages in unlawful conduct, poses a safety threat, or violates posted policies. Service refusal may occur without refund and without further explanation. 11. Responsibility for Agency Receipt Confirmation Following fingerprint submission, clients are responsible for verifying that their results were received and accepted by the designated agency. Ohio Security & Investigations, LLC and 10-8 Protective Services, LLC assume no responsibility for missing submissions, processing delays, system errors, or agency-side miscommunications. 12. Jurisdiction and Governing Law Provision This document and all services provided are governed exclusively by the laws of the State of Ohio. Clients agree that any dispute, claim, or legal proceeding shall be adjudicated solely within the courts located in Mahoning County, Ohio, and that venue challenges or jurisdictional objections are waived upon acceptance of services. 13. Comprehensive Hold Harmless and Legal Indemnification Clause By proceeding with fingerprinting or background check services, entering the premises, or communicating with representatives of Ohio Security & Investigations, LLC or 10-8 Protective Services, LLC, the client voluntarily assumes all risks and irrevocably waives any right to pursue legal action for outcomes resulting from the fingerprinting process. This includes procedural delays, submission rejection, illness, injury, financial loss, reputational impact, denial of licensure, or agency decisions based on reported data. Furthermore, the client agrees to fully indemnify, defend, and hold harmless both companies—including all owners, officers, employees, agents, contractors, successors, affiliates, and legal representatives—from any and all claims, liabilities, damages, penalties, judgments, fees, legal costs, and third-party actions arising directly or indirectly from their conduct, presence, participation, or any misrepresentation during the fingerprinting process. Acceptance of service confirms this indemnification as enforceable under applicable law and affirms that no oral modification shall override its validity.
