LIVESCAN FINGERPRINTING BOOKING FORM

Background Check Scheduling

Appointments are required; walk ins are not accepted.
Hours: Monday 12 to 4 PM, Tuesday to Friday 8 AM to 4 PM. Closed weekends.
After-hours fees are additional: $100 on weekdays, $150 on weekends.
Fingerprinting is not available for law enforcement, CCW, or CDL. Please contact your local sheriff or the Ohio BMV for these services.
Payments: 4.5 percent credit card fee and $50 returned check fee apply.
